Circadian Rhythms and the Cost of Shift Work

Nearly every cell in the human body follows an internal clock, known as the circadian rhythm, which regulates processes including sleep, hormone release, body temperature and digestion over an approximately twenty-four-hour cycle. This clock is primarily synchronised by exposure to light, particularly daylight, which signals to the brain's central pacemaker, located in a small region called the suprachiasmatic nucleus, what time of day it is. For people who work regular daytime hours, this system generally functions smoothly, aligning the body's internal processes with the external environment. Shift workers, however, particularly those on rotating or night schedules, are frequently required to be alert and active precisely when their circadian rhythm is signalling that the body should be winding down for sleep, and to sleep during daylight hours when the body's internal clock is promoting wakefulness. Research consistently links long-term shift work to a range of negative health outcomes beyond simple tiredness, including a higher risk of cardiovascular disease, metabolic disorders such as type 2 diabetes, and certain digestive problems, outcomes generally attributed to the chronic disruption of hormonal and metabolic processes that are supposed to be tightly coordinated by the circadian system. Cognitive performance and mood are also commonly affected, with many shift workers reporting persistent difficulty concentrating and increased irritability, particularly during the initial weeks of adjusting to a new schedule. Employers and researchers have experimented with various strategies to reduce this burden, including carefully designed rotation schedules that shift in a forward, rather than backward, direction, which appears easier for most people's circadian rhythms to adjust to, as well as controlled exposure to bright light during night shifts and strict avoidance of light during the day before sleep. While such measures can meaningfully ease the transition for many workers, researchers generally agree that they cannot fully eliminate the underlying physiological mismatch inherent in working against the body's natural clock.
